PE Credit Form
For the graduating class of 2024 and beyond
Students may earn 1/4-credit of the physical education requirement for each full season of participation in approved interscholastic or intramural athletic competition. The total credit earned shall not exceed one-full credit. Fill out the form below and submit it to your school counselor.
