Home Language Survey • Parent Language Questionnaire
Upon initial enrollment in school, a Home Language Survey must be completed for every student. This survey will be used to determine which students should be assessed for English Language Proficiency. This form will not be used for immigration matters or reported to immigration authorities.
Note to enrollment personnel: The completed form should be placed in the student's permanent file and a copy of this form should be sent to the ELL Program Office.
