Medications at School

 

School Medication Policy – Requirements for All Medications & Health Action Plans

All required medication forms must be completed before Health Services can administer any medication at school. Medication administration forms for the upcoming school year must be dated July 1 or later , as new forms are required each school year.

Bringing Medication to School

  • All medication must be brought in by a parent or guardian. Students may not transport medication to or from school.
  • Medication must be counted with a parent/guardian present, and the Medication Inventory Log must be signed before medication can be administered.
  • We cannot accept medication that is not in the original pharmacy‑labeled container. Pharmacies can provide a “school bottle” upon request.
  • Over‑the‑counter (OTC) medications must also be in their original container.
  • All medication must be picked up by a parent/guardian on or before the last student contact day or it  will be disposed of.

Prescription Medication Requirements

Long‑Term Prescriptions

Medications taken for more than 10 days for chronic health conditions (e.g., ADD/ADHD).

·       Medical prescriber signature required

·       Parent/guardian signature required

Short‑Term Prescriptions

Medications taken for less than 10 days for acute conditions (e.g., antibiotics for infections).

·       Parent/guardian signature required


Required Health Action Plans for Chronic Conditions

To ensure a safe and healthy school experience, students diagnosed with chronic health conditions that may require treatment or emergency care at school must have a current Health Action Plan on file.

All Action Plans must meet the following requirements each school year:

·       Completed by a licensed healthcare provider

·       Signed by a parent/guardian

·       Signed by the student when self‑carry is appropriate

·       Updated annually or whenever medical orders change

·       All medication and required supplies must be provided by the parent/guardian
