Name Change Packet
Use this packet for name changes, or use the individual forms below for the following:
Submit this form and your official transcripts with posted credits as a complete packet. For electronic transcripts please refer to the Transcript Submission instructions from the website.
Certified employee requests: please submit completed form to the Human Resources Office no later than March 1.
Classified employee requests: please submit completed form to the Human Resources Office no later than April 1.
This form serves as official notice in lieu of or in addition to written notice.
This form should be completed for each day/week for which overtime is requested. The request should be submitted to your supervisor BEFORE working over and above your regular workday/week.
Supervisor completes the web form that will be submitted for processing through Adobe Sign. Contact your HR Coordinator if you need assistance completing the form.

This form should be completed by the department in charge of employment records where prior service was rendered. This information is required for salary placement and must be date stamped in the FNSBSD Human Resources Department within six (6) weeks from the date of hire.
