PHYSICALS - Physical Examinations for Athletic Activities and Sports: (Refer to Board Policy 955.24)
Any student in grades 9 through 12 who participates in athletic activities, both interscholastic and intramural, including cheerleading, must have a physical examination within the prior 18 months and a signed parent consent form on file prior to beginning his/her first team practice of the school year.
Any student in middle school, who participates in athletic activities including tryouts, both interscholastic and intramural, must have a physical examination and a signed parent consent form on file prior to participating.
This physical must be performed by a practicing physician, and advanced nurse practitioner (ANP), a physician’s assistant (PA) or a military medical examiner, and the student’s parent or legal guardian certifying that he or she has submitted to a history and physical examination within the prior 18 months. The statement must certify that, in the opinion of the examiner, the student is physically fit to participate in strenuous physical activities.
Any elementary student participating in interscholastic or intramural sports must have a signed Parent's Consent form on file prior to participating in the activity.
This physical must be performed by an M.D., a D.O., A.N.P., or a P.A. under the supervision of an M.D. or a D.O.
