Submit this form and your official transcripts with posted credits as a complete packet. For electronic transcripts please refer to the Transcript Submission instructions from the website.

Job Share Request Form
Certified employee requests: please submit completed form to the Human Resources Office no later than March 1.
Classified employee requests: please submit completed form to the Human Resources Office no later than April 1. -
Notice of Resignation / Retirement
This form serves as official notice in lieu of or in addition to written notice.

Overtime Authorization Form
This form should be completed for each day/week for which overtime is requested. The request should be submitted to your supervisor BEFORE working over and above your regular workday/week.

Verification of Service
This form should be completed by the department in charge of employment records where prior service was rendered. This information is required for salary placement and must be date stamped in the FNSBSD Human Resources Department within six (6) weeks from the date of hire.

Change of Status Form
Use this form for:
- Administrative Leave
- Professional Leave
- Certified Staff – Personal Leave in Excess of 6 Days
- Short Term Leave Without Pay (LWOP) – Less than 30 days
- Long Term Leave Without Pay / Leave of Absence – 30 days or more
- Military Leave
- Childcare Leave
Submit this completed form and related documentation to labor.relations@k12northstar.org.
Employees are still required to input all leave into TCP and/or AESOP. -
Sick Leave Transfer Form
Per AS 14.14.107, a certificated school district employee who changes employment from one school district to another district, or from a school district to the department, or from the department to a school district, may transfer all of the cumulative sick leave to the new employer. It is the responsibility of the employee to notify the new employer, within 90 days of commencing work, of the number of days to be transferred.

Limitation of Assignment
Purpose of this form:
- Advise Human Resources of concerns
- Request to restrict employee from working at this location (Decision made by Human Resources)
Sub/Temp Complaint Matrix
All Teachers/Building Administrators may report their concerns about a Substitute Teacher/Temporary Employee in the job that was assigned at that school. The Limitation of Assignment form is to be completed and returned to the Sub/Temp Senior HR Technician within 48 hours of the incident. If student safety is a concern report the incident immediately to the Assistant Superintendents and either the HR Executive Director or the Employee Relations Specialist.
