School Sports and Clubs Information

  • PHYSICALS-Physical Examinations for Athletic Activities and Sports: (Refer to Board Policy 955.24)

    Any student in grades 9 through 12 who participates in athletic activities, both interscholastic and intramural, including cheerleading, must have a physical examination and a signed parent consent form on file prior to beginning his/her first team practice of the school year.

    Any student in junior high/middle school who participates in athletic activities including tryouts, both interscholatic and intramural, must have a physical examination and a signed parent consent form on file prior to participating.

    Any elementary student participating in interscholastic or intramural sports must have a signed Parent Consent form on file prior to particiating in the activity.

    This physical must be performed by an M.D., a D.O., A.N.P., or a P.A. under supervision of an M.D. or a D.O.
