Planet HS

Planet HS is where students and parents complete all their athletic forms and upload their physicals to be able to participate in a sport.  

The website is planeths.com

Both Students and Parent/Guardians need to complete about 9 forms on Planet HS.

If you have never signed up before, have the student sign onto planeths.com.  Then they will get the option to invite a parent/guardian through email or text.  The parent can then use the link to sign up on planeths.com.  Both students and parents need to select Athletic Forms.  There are about 9 forms that need to be filled out, both by parents and students.  Make sure you always click submit at the end of the page.  You will also need to take a picture of your physical that has been done within the last 18 months, and upload it on planeths. 

To verify that both students' and parents' forms are complete, click on athletic forms and scroll down to the bottom of the page. If you have both completed all the forms, it should show each form as complete. *You do not need to do the optional form.
